In terms of position I normally use the one facing forwards (https://www.shoulder-pain-explained.com/images/pulley-shoulder-flexion-forwards.jpg) or the one shown in the blog. As for frequency, reps and sets I am probably suggesting little and often throughout the day within their pain levels as this is fairly passive in nature. If the pulley is somewhere where they can leave it, it is a good reminder that every time they pass that door they do their exercise. Not the best exercise but some patients seem to love it.

Yes I'm really liking the blog, it's a good way to keep tabs on shoulder-specific physio. And thanks for that - I'll have to give it a go myself!
